Just checked mine after a year and a half steaming almost everyday. I use good water and purge and wipe (twice) after each steam.
My tip unscrewed easily with my bare hands and the tube and holes looked spotless.
I think it's just a matter of good routines and muscle memory.
Purge, wipe, Purge Wipe.

Talked to Clive, I ordered the correct tips, the wand is two pieces they said and I have the adapter piece stick to the wand steam tip.
I am religious about purging and wiping at least twice after milk, every single time, I guess 14 months of it just got it stuck together...

I've always unscrewed the tip a half turn after purge and wipe and gently snug up.
Repeated heating and cooling a threaded parts can cause them to loosen or weld.

Does that mean you unscrewed this adapter from the wand when you removed the tip? If so, it should be easy to remove the adapter from your 2-hole tip and insert it back either into the wand or into the 4-hole tip... I'm not sure why ECM changed the threading on the steam wand via an adapter that you risk unscrewing when changing steam tips. Seems like a design flaw to me.
