Spella Caffe in Portland roasts its own outstanding blends, and pulls all shots with a lever. As mentioned above, Mr. Spella has a cart at SW 9th and Alder, but now he also has a tiny indoor shop on SW 4th.

Just returned from Morocco where my wife was playing a concert in tetuan and to my surprise every coffee shop there had a lever machine. Mostly gaggias and Astorias. The town had an incredible coffee culture. Though actually only men were in the coffee shops. Even the smaller convenience stores/stalls had levers. And the espresso excellent. Best one was on the pedestrian walk on the right just before the royal palace.

Mizuna, Spokane, WA
While enjoying the vegetarian fare at Mizuna in Spokane, I noticed that they have a Cafethema single group. I had not seen one in commercial operation prior to today. Since Atticus is next door, I went there for my coffee however.

Two entries from Brooklyn, NY:
Oslo Coffee - the location at 328 Bedford Ave has a lever machine and pulled me a good shot once.
Blue Bottle - the cafe/roastery at 160 Berry St has a Marzocco and a lever machine.
Both cafes are within a short walk of one another.

(A very pretty machine)
Gusto Crema Blend by Cafe Umbrio, Seattle, WA
The barista flushed the group just before pulling my shot.
Results: Thin with little creme, and not much flavor, Bitter but at
least not burnt.
Another uninspiring shot from a cafe that could do better.
I think my home levers have really spoilled me. I want richness and
flavor!
