My classic move is to have the switch on from puffing the previous shot, adding beans, and closing the cover with the basket and funnel nowhere near the chute.

Oh yass. I prep the basket outside the portafilter, and periodically forget to insert it. Shot tends to be kinda weak.

Then there's the time I poured orange juice into my "cappuccino". Latte art definitely suffers.
