For a straight pitcher I was taught to put the tip in the middle and at about 45 degree angle. I use a toroid pitcher so I just put the tip in the middle directed straight down.

Sometimes, especially depending on the amount of milk, and even sometimes also depending on the amount of stretching, I need to put the tip pretty far down into the milk to get whirlpool. And sometimes you have to wait a few seconds for the whirpool to get going.
