It's likely to be the bottom seal - a tear will allow coffee to bypass it, and possibly be sucked back into the boiler when the machine cools, of if you create a vacuume by steaming milk.
Occasionally they dry out during shipping, then can break.
The piston removal tool will make your life very simple. Most likely once you've got good seals on it they will last a while and the problem will disappear.
