I reformatted the OP to break down all the information posted in this thread and other threads I've found by city. I also added links to cafe web sites wherever possible.

Art Java in Montreal on de Maisoneuve switched to JJ Bean Coffee a little while back.. I can't confirm that all the Art Java's in the city switched but it wouldn't surprise me.

I'm jealous of Montreal again with the addition of Le Café & La Plume which features Verve. Looking forward to visiting.

Sorry to hit the same nail in the Toronto vs Montréal coffin:

From Marc and portamento, in A case for home roasting?:

Being in Montreal, you have access to several "3rd wave" cafes that carry coffees from some of the best roasters in North America. Why not buy some whole bean around town and support your local retailers instead of paying hefty shipping fees?

Here's what I came up with, but I'm sure there are more:

Cafe Myriade, serving 49th Parallel, Ritual Roasters, Phil & Sebastian, etc.
Caffe in Gamba, serving Intelligentsia, Zoka, Novo, Vergnano, 49th Parallel
Cafe Veritas, serving 49th Parallel
Caffe Art Java, serving JJ Bean (previously Gimme! Coffee)
Flocon Espresso, serving Stumptown
Cafe Neve, serving Metropolis

Cafe Myriade, serving 49th Parallel, Ritual Roasters, Intelligentsia
Caffe in Gamba, serving Intelligentsia, 49th, counter culture, metropolis, kickapoo, klatch, St-Henri
Cafe Veritas, serving 49th Parallel
Caffe Art Java, serving JJ Bean (previously Gimme! Coffee)
Flocon Espresso, serving Stumptown and Metropolis
Cafe Neve, serving Metropolis
St-Henri Micro-torréfacteur serving their own beans.

Myriade prices are competitive and very similar to the one from the roaster.
Gamba is 20$ a bag (few exception)
Flocon is 15$ for 12oz bag of stumptown
St Henri sells their own been to about 15$ for 12oz.

All of them have a great turnover, some exception for Gamba less popular choices

You should add that Caffe Art Java on Mont-Royal still sells Leftist. I bought some fresh stuff from them last week.

Also:

Pikolo (Parc & Sherbrooke) sells Phil & Sebastien
You missed the Cafe Plume selling Verve

Also Myriad doesn't really always have Intelligentsia. They have also alluded to a "Coming soon" for Cafe Grumpy coffee. We'll see.
