Unless something got into the piston cylinder to tear it, I suspect it got dried out and stuck to the wall. Was the machine either, a) unused for a while, or b) the lever operated when cold? The main lubricant for the seals is water, and this only enters the group when the machine has come up to brew temperature. As noted, don't used the lever before the machine is warmed up.
For what it's worth, I've only changed (and lubed) seals once in over 5 years, and only that after dry storage.
