Some medical uses of caffeine are:
Caffeine citrate: Treatment of idiopathic apnea of prematurity (and some physicians would say is preferred to the other methylxanthine theophylline)
Caffeine [OTC labeling]: Restore mental alertness or wakefulness when experiencing fatigue

Unlabeled/Investigational:
Caffeine: Post spinal puncture headache treatment (but caffeine is given intravenously (IV) along with sodium benzoate); CNS stimulant (as mentioned above) ; diuretic; augmentation of seizure induction during electroconvulsive therapy (ECT); migraine headache abortive treatment (e.g. caffeine is in excedrin migraine); athletic performance enhancer ( e.g see "Cycling Weekly Health & Fitness" Spring 2009 issue p. 92-95; that article suggests 3-9 mg caffeine per kg body weight one hour prior to exercise; per that article as of 2004, caffeine is not barred by WADA (World Anti-Doping Agency))

Caffeine also has pain-killing properties. I've found that it helps to take the edge off phantom-limb pain —slightly; however it's enough to discern a distinction.

Ahhh...I sometimes wonder how nice it would be if espresso had no caffeine. It could then be consumed ad infinitum with little regard to the consequences. We would all be able to enjoy dozens of shots per day which would likely result in killer barista skills in a matter of a few weeks.

Alas, 'tis not so...so we must "pace" ourselves and enjoy that little beverage only a few times per day.

Well.. some of us... can consume pretty much as much as they want, with little to no effect. Ahh the joys of drinking coffee since the 2nd grade. Er... For some of us :)
