When served with club soda, what is the order of consumption? Or does the club soda get mixed into something?

I believe that the intended use is for one to clean their palate before drinking the espresso. I think this is also why you also receive sparking water with a regular espresso.

Traditionally you take a sip of the soda to clear your palate. Then a sip of the espresso. If the shot is good you finish the soda and then the espresso to let it linger on your tongue. If the shot isn't good then you quickly down it and then wash it down with the rest of the soda so it doesn't longer on your palate.

I wouldn't have known about this except that Jared Truby and Chris Baca mentioned it on one of their recent episodes of the Cat & Cloud podcast. They served the two drinks balanced on one saucer.

When I visit a new shop and want to test it out I've always ordered an espresso shot and a cappuccino. Gives me a chance to check out the place with a broader perspective. I like the idea of the 1 & 1, especially when first investigating a cafe - wish more places had it available.
