Couple of things,
1. Your shot looks a bit pale, not much crema that the milk can stay under before pouring the art.
2. Try to pour a bit of milk from a distance so the milk gets under the crema, then mix with a spoon the coffee and milk.
3. You NEED to be in the CENTER of your cup !!!
4. Pouring too fast creates a big white cloud, you need to find the right pace.
5. Don't wiggle until you see white on top of you coffee.
6. Texture of the milk is really important.

I would pour more in the middle and closer. That will mix everything up nicely so the crema is not as important. You can even practice with hot coco instead.
