I'll chime in here with my defense of the toothpick plug... Having removed a plug once to change which hole I had plugged, I can tell you that, at least in my case, dislodging is a pretty unlikely thing. Mine took about 5 minutes to get out, and it came out in pieces. It was in there solid. Perhaps making sure it's in really well can reduce the possibility of it coming out when you don't want it to.

Here is a quick fix replacement tip for the Pavoni three hole steam tip. I purchased some stainless acorn nuts from my local hardaware store and drilled a hole with my drill press. I have another with two forward facing holes. Microfoam has never been easier.
